HealthMapRx has evolved from more than a decade of research by the American Pharmacists Association Foundation, this community pharmacy-based program had its beginnings in 1996 in Asheville, North Carolina, and continues on today as HealthMapRx.  The success of this model has taught savvy business leaders that health care can be an investment in well-being rather than an expense for sickness. HealthMapRx empowers health plan beneficiaries to successfully navigate the complexity of living with a chronic disease. Using a pharmacist allows for easy accessibility to a health care provider to help coach along the way. 

HealthMapRx is a patient-focused collaboration between your employee, a community pharmacist and his/her health care team. The goal: self-care education. In private, face-to-face counseling sessions with community pharmacists, employees learn how to better manage their chronic conditions (diabetes, high blood pressure, cholesterol) and reduce associated risks.

• Employees (and their families) can better manage chronic conditions such as diabetes as well as associated cardiovascular risks, including hypertension and hyperlipidemia. Thanks to their pharmacist-coach, employees receive the personalized attention they need to take control of their disease and make positive lifestyle choices.
