Employers today are far too familiar with the statistics regarding rising healthcare costs and the main culprit— chronic disease, with its associated high treatment costs and lost productivity. The HealthMapRx℠ program provides today’s leading employers with the opportunity to combat these issues by empowering employees to successfully navigate the complexity of living with a chronic disease.
HealthMapRx works with employers to offer consumer incentive programs that focus on patient self -management education and techniques to help patients with chronic conditions improve health outcomes. The program matches health plan beneficiaries with community pharmacist “coaches” who provide hands-on education, while monitoring and evaluating the employees’ health improvements. Through one-on-one counseling sessions with pharmacists, employees can learn how to better manage their chronic conditions (diabetes, high blood pressure, cholesterol) and reduce associated risks.
